Job Description

ROTATING TOUR

SUMMARY OF DUTIES AND RESPONSIBILITIES

Under the direction of the Clinical Nutrition Manager and the Plant-Based Lifestyle Medicine Program Director, provides Medical Nutrition Therapy to adult patients in the Plant-Based Lifestyle Medicine Program. The dietitian follows Hospital and Departmental Policies and Procedures and participates in all interdisciplinary activities as a member of the health care team.
  • Provide nutrition services for a facility-based Plant-Based Lifestyle Medicine Program.
  • Document care plans, consultations, recommendations, and follow-up interventions in the electronic medical record.
  • Participates in the formulation and implementation of procedural guidelines for Medical Nutritional Therapy, which include a nutritional assessment based on diet history, clinical data, and educational and social needs.
  • Develops, implements, and coordinates ongoing quality assurance measures to improve nutritional services.
  • Review clinical and laboratory data to formulate a plant-based nutrition care plan.
  • Educate and counsel patients on the nutritional management of various chronic diseases with an emphasis on prediabetes, type 2 diabetes, obesity, hypertension, and atherosclerotic heart disease.
  • Provides extensive education and counseling on plant-based nutrition as a member of the Plant-Based Lifestyle Medicine team, using motivational interviewing techniques.
  • Consult with other team members as necessary, including physicians, nurse practitioners, health coaches, social workers, community health workers, and program coordinator.
  • Participates in hospital-wide health fairs, staff health challenges, and community-based outreach programs.
  • Create culturally sensitive educational materials and facilitates educational groups/shared medical appointments for program participants with attention to literacy levels and diverse patient population needs.
  • Collaborate with other institutions and organizations on projects and courses related to nutrition and lifestyle medicine.
  • Offer nutrition classes, cooking demonstrations, and lectures as needed to patients, employees, students, and clinical staff.
  • Provide instruction, consultation, and supervision as needed to medical students, residents, fellows, dietetic interns, and volunteers.
  • Participate in the development and implementation of quality improvement activities as assigned.
  • Maintain continuing education in nutrition (including plant-based nutrition), lifestyle medicine, clinical dietetics and related areas as appropriate in order to maintain current knowledge and skills in the practice of clinical nutrition.

Minimum Qualifications

Qualification Requirements:

For appointment to Assignment Level I
1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or,
2. Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Level II
1. Registration or eligibility for registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Levels III and IV
1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility:
Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
