Job Description

Program management of projects in the Digital Health portfolio aimed to improve primary care delivery across Health + Hospitals. This is a hybrid role with expectations for the candidate to be on site 2-3 days a week.

General tasks and responsibilities will include:
  • Manage, oversee and facilitate clinical, business, and regulatory activities around primary care analytics, with particular attention, but not limited, to work around Digital Health (telehealth, remote patient monitoring, patient portals, patient devices, etc.)
  • In collaboration with others, design system-wide standards and guidance for above analytics as well as develop meaningful ways to measure and report on relevant metrics
  • Innovate and support Digital Health strategies and guidance
  • Create training materials and train staff on program initiatives
  • Provide elbow to elbow support for programs on-site
  • Build metrics, pull and produce reports on outcomes data and evaluate the efficacy of improvement.
  • Collaborate with partners from Managed Care, Quality, Finance, and Epic to improve Digital Health tools and projects across the enterprise.In collaboration with others, develop best practices for team-based care.
  • In collaboration with others, support development of reports and tools to improve primary care, validate this data for integrity, and analyze/visualize data in user-friendly way for enterprise-wide audiences
  • Manage administrative tasks such as licensing, grants, contracts with vendors, and monitoring utilization related to panel management or related ambulatory care initiatives
  • In collaboration with others, evaluate the efficacy of initiatives within the Office of Ambulatory Care
  • Assist with other Office of Ambulatory Care activities as needed


Minimum Qualifications
1. A Baccalaureate Degree from an accredited college or university in a specialization discipline consistent with project content; and,
2. Four years of substitutable and relevant health care experience related to the objectives of the project; or,
3. A satisfactory equivalent combination of experience, training and education.

Department Preferences

  • Master's degree in public health, computer science or other relevant study
  • 3-5 years within healthcare operations or relevant support services
