Job Description

PURPOSE OF POSITION

Director of Nursing (DON) for Service Delivery - Clinical Services will be responsible for providing oversight of a team Assistant Director of Nursing as well as nursing staff who provide oversight to a clinical team in the coordination and rendering of care delivery in the Certified Home Health Agency (CHHA). The DON for Service Delivery - Clinical Services will be responsible for the delivery of competent, effective and safe clinical services, including but not limited to skilled nursing, as well as other interdisciplinary services. In addition, the DON for Service Delivery - Clinical Services will engage in efficient advisement, coaching and mentoring strategies to the members of the care team in order to meet clinical and professional goals. The DON for Service Delivery - Clinical Services will stay abreast of industry trends and regulatory guidelines that govern the home health care team and clinical practice.

The Director of Nursing for Service Delivery and Clinical Services is part of the leadership team that will focus on ensuring that the home care department, maintains a collaborative relationship with the acute and post-acute care facilities within the health system.

Duties & Responsibilities

ESSENTIAL DUTIES AND AREAS OF RESPONSIBILITIES
1. Develop and review metrics that lend to the growth of the CHHA and management of staff productivity and work performance.
2. Hires, trains and develops staff based on the growth of the program and operational needs of the organization.
3. Oversees the daily functions and activities of the CHHA Nursing Service Delivery team.
4. Liaises with acute and post-acute care discharge planning teams to ensure transition goals for patients are met.
5. Thinks strategically in planning for referral sources, patients and the home care department to improve on the quality of care being rendered.
6. Assures adequate supply of qualified staff based on operational needs and productivity targets. Assists with recruiting, hiring and orienting all clinical staff.
7. Provides one to one mentoring and education to direct reports as needed.
8. Provides adequate opportunity for education and ensures that all staff meets mandatory educational and in-service requirements on a timely basis.
9. Contributes to the overall operations and strategic planning for the growth of the Certified Home Health Agency.
10. Oversees the clinical staff assigned to the various CHHA Teams to establish and maintain positive, productive relationships with facility stakeholders and to support effective patient support.
11. Thinks strategically for our patients and caregivers, leveraging technology to streamline processes and improve quality of care.
12. Works closely with Directors of Clinical and Community-Based Services and TCNs to ensure compliance with home care regulatory guidelines, review "safe" discharge planning, payer limitations and restrictions.
13. Assumes on-call responsibilities as required and serves as back up to Assistant Directors of Nursing (ADN) for Service Delivery - Clinical Services.
14. Manages schedule of ADNs for Service Delivery - Clinical Services and ensures team coverage in the absence of the ADN for Service Delivery - Clinical Services.
15. Ensures that staff is counseled appropriately and timely with regards to work performance, attendance, punctuality, patient care quality and productivity issues.
16. Conducts employee performance evaluations and assists with the development of new job descriptions as needed.
17. Reviews and updates current job descriptions annually, and when necessary.
18. Ensures availability of structured onboarding plans for all assigned personnel.
19. Facilitates positive interpersonal relations among staff with a focus on promoting and modeling ICARE values in all engagements.
20. Ensures that the agency is following all Federal, State and accrediting body regulations.
21. Participates in short/long-range strategic planning and implements specific and targeted measures for growth.
22. Establishes workflows as needed to stream-line processes that will enhance operations and work performance.
23. Contributes to the development of marketing strategies and participates in programs aimed at improving the visibility of the home care department.
24. Strengthen relationships with new and existing patient care providers by daily or weekly follow-up.
25. Conduct ongoing assessments to ensure ADN for Service Delivery - Clinical Services presence, engagement and interaction within their assigned teams and direct reports and make changes when needed.
26. Conduct regular meetings to facilitate the dissemination of information and maintain the team's ongoing connection within the Community Care network.
27. Assists with community relations and community education.
28. Travel between multiple post-acute and acute care sites required daily.

Minimum Qualifications

Qualification Requirements:
1. A valid New York State license and current registration to practice as a Registered Professional Nurse; and
2. Master's degree from an accredited college or university in Nursing Administration or a related field and four (4) years of high-level responsible experience in nursing administration, three (3) years of which must have been in a hospital; or
3. A satisfactory equivalent combination of education, training and experience. However, all candidates must be licensed and currently registered to practice as a Registered Professional Nurse in New York State.

Additional Qualifications

KNOWLEDGEABLE IN:

Demonstrated knowledge regarding Federal, State and local regulations and JCAHO standards for Home Care.

Excellent verbal, written, organizational and analytical skills.

YEARS OF EXPERIENCE:

Minimum of 3 years high-level responsible experience in nursing administration

Minimum three 2-3 years of experience in home care

Minimum of 1 to 2 years' experience in an acute care or in-patient post-acute care setting.

Minimum 1 to 2 years' experience in discharge planning preferred.

Minimum 1 to 2 years' experience in business development preferred.

Previous experience using various computer/mobile software applications: i.e., Outlook, Word, Excel, PowerPoint, Visio, OneNote.
