Salary: Up to $85k - $95k

Responsibilities will include:

• Conducting thorough patient assessments and creating personalized care plans to ensure our residents feel at home in their new environment.
• Establishing comprehensive policies and objectives for the Social Services Department, promoting a culture of excellence and compassion.
• Resolving operational issues with finesse and efficiency, collaborating seamlessly with other departments to ensure the highest quality of care.
• Continuously assessing evolving patient needs and providing families with essential information to support their care planning journey.
• Engaging residents and families in the development of social service programs, fostering a sense of community and belonging.
• Offering innovative solutions to social and environmental challenges, including financial assistance and discharge planning, while collaborating with community agencies for specialized support.
• Regularly interfacing with administration, medical, and nursing staff, as well as other stakeholders, to ensure cohesive care delivery.
• Expertly interpreting residents' social, psychological, and emotional needs, facilitating effective communication with the care team.

Minimun Requirements:

• A minimum of 3 years of experience in Skilled Nursing Facilities, demonstrating a commitment to excellence in care.
• Exceptional communication and interpersonal skills, with a passion for serving others.
