Position: Social Services Assistant
Location: Anchor Care and Rehabilitation Center
Shift: Part-Time, Monday - Thursday, 9 AM - 5 PM
Pay Rate: Up to $18 per hour

In this role, you will play a vital part in maintaining the well-being of our residents by providing essential clerical support and helping to coordinate various social services initiatives. This is an excellent opportunity for someone with strong customer service skills and a passion for working with people in a healthcare setting.

Key Responsibilities:

• Maintain organized and accurate records within the department.
• Assist residents and families in navigating benefits from community and government agencies.
• Conduct thorough admission assessments for new residents.
• Oversee and update the weekly discharge calendar and UR tracking documents.
• Schedule and coordinate care plan meetings, including ADHOC and discharge planning.
• Respond to emails and phone calls promptly, ensuring timely follow-ups on important matters.
• Provide hands-on assistance to residents and families regarding their care needs.
• Serve as a key point of contact for residents, families, and interdisciplinary team members.

Qualifications:

• Associate degree in Human Services, Sociology, or a related field.
• Proficiency in Microsoft Office
• Strong customer service and communication skills.
• Exceptional organizational and data management abilities.
