Job Description

Under general supervision, with moderate latitude for the exercise of independent judgment, in addition to performing the typical tasks described in Assignment Level I above, performs the following typical tasks:

The Social Worker provides client evaluation and social work services to DV Shelter clients. They will support shelter caseworkers and social workers in the screening, assessment and referral of clients for clinical mental health services provided by Heath and Hospital. Social Worker II-IV may provide and coordinate services at shelter designated locations, the Family Justice Center, or via Tele-Health.
  • Provide adult, children, and group socio-psychotherapy including crisis and grief counseling at required level to those impacted by IPV and referred to care.
  • Coordinate care for clients after assessments with DV shelter and H+H clinical team.
  • Collaborate with center staff and on-site service agencies related to clients' clinical needs. Assist in facilitating referral for psychiatric care when appropriate.
  • Perform administrative duties including documenting care metrics to ensure we are meeting Central Office targets.
  • Responsible for monitor all patients in case load from referral to intake to post-referral treatment to ensure good patient care.
  • Present intake data at weekly clinical meeting.
  • Maintains and competes statistics of case activities as outlined or requested by Director.
  • Ensures patient records are stored securely, confidentially, and compliant with HIPAA standards. Interfaces with hospital Medical Records department as necessary.
  • Attend all trainings and departmental meeting as requested.
  • In the temporary absence of the supervisor may perform duties if requested by Director.


Minimum Qualifications

For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.

Department Preferences

  1. Supports and trains caseworkers and DV social workers
  2. Develops instructional materials and techniques
  3. Experience with Domestic Violence and/or trauma population. Strong interpersonal and communication skills and ability to work well in a multidisciplinary team setting. Familiarity with high-risk patients and cultural sensitivity in community settings. Familiarity with high-risk populations and community settings, demonstrated interest in maternal mental health. Administrative and research experience a plus. Child treatment also plus.
  4. Two (2) years' experience in a Hospital setting, preferably in Behavioral Health Services
  5. Bilingual Spanish preferred. Other languages a plus.

CERTIFICATION(S)/NYS LICENSE(S):
  • Licensed Master of Social Work (LMSW) license,
  • Licensed Clinical Social Work (LCSW) License or a
  • Limited Permit issued by the New York State Education Department

EDUCATIONAL LEVEL:
  • A valid Licensed Master of Social Work (LMSW) license, Licensed Clinical Social Work (LCSW) License or a Limited Permit issued by the New York State Education Department. Employees working with a Limited Permit must obtain their LMSW or LCSW with one year of the date of appointment
  • Two years background at responsible management and administrative levels; or, An equivalent combination of training, educational background and experience in related fields and educational disciplines; and
  • General knowledge of policies and programs utilized in the operation of a health care facility, ability to assimilate and analyze data, to recommend action based on analytical findings, and to assist in the solution of complex problems in health care administration and management.
