Travel: 50%-60% of your time will be traveling to patients homes. The remainder will be telephonic visits.
The travel area will be Kansas City and West of the city. Must be able to travel up to a 50 mile radius one way.
Schedule: Monday - Friday, 8:00 a.m. - 5:00 p.m.

Primary Responsibilities:
- Traveling to patients homes
- Make outbound calls and receive inbound calls to assess members' current health status
- Identify gaps or barriers in treatment plans
- Provide patient education to assist with self-management
- Make referrals to outside sources
- Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
- Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels

Required Qualifications:
- Current, unrestricted RN license in the state of employment
- 3+ years of experience in a hospital, acute care or direct care setting
- Ability to type and navigate a Windows based environment
- Kansas RN Compact License
Preferred Qualifications:
- BSN
- Certified Case Manager (CCM)
- Demonstrated background in managed care
- Case management experience
- Experience or exposure to discharge planning
- Experience in utilization review, concurrent review or risk management
- Experience in a telephonic role
