The Care Coordinator, MA (Telemedicine Programs Specialist), plays a vital role in supporting and optimizing telemedicine services, including Annual Wellness Visits (AWV) and Initial Preventive Physical Exams (IPV), for patients residing in Skilled Nursing Facilities (SNF). This fully remote position is responsible for managing and coordinating telemedicine care programs by extracting and analyzing patient data from the PCC Electronic Medical Records (EMR) system. The Care Coordinator ensures effective and efficient delivery of care through telemedicine services by acting as the central link between patients, SNF staff, and the remote clinical team of a third-party provider.

Responsibilities:
  • Extract and manage patient data from the PCC EMR system, focusing on the coordination of AWV and IPV telemedicine services.
  • Serve as the primary point of contact between the remote clinical team and SNF staff, ensuring seamless communication and data flow.
  • Collaborate with remote healthcare providers to facilitate the creation and implementation of patient-centered care plans tailored to telemedicine frameworks.
  • Educate and guide patients and SNF staff on the telemedicine process, focusing on optimizing chronic condition management and preventive care.
  • Advocate for patients' needs by ensuring they receive comprehensive and effective care through the telemedicine programs.
  • Monitor and assess patient progress within telemedicine programs, coordinating with the clinical team to make necessary adjustments to care plans.
  • Maintain accurate and up-to-date patient records, ensuring timely and effective communication of health data between the SNF and the remote clinical team.
  • Support continuous quality improvement by identifying opportunities to enhance the telemedicine services provided.

Qualifications:
  • Certification as a Medical Assistant or in a related healthcare field.
  • Experience with telemedicine programs, specifically AWV and IPV, with a solid understanding of care coordination within these settings.
  • Proficient in electronic medical records (EMR) platforms, particularly PCC, with a high level of digital literacy.
  • Strong organizational and multitasking skills, with the ability to manage detailed patient data and complex care programs.
  • Knowledgeable in medical terminology, chronic condition management, and the requirements of telemedicine care delivery.
  • Excellent communication skills, with the ability to effectively coordinate between patients, healthcare providers, and SNF staff.
