The permanent/full-time Health Record Technician I will be directed by the Health Record Technician II (Supervisor) in the Health Information Management Department (HIMD) of the Department of State Hospitals - Atascadero (ASH). Under close supervision, works with relatively structured records and learns the principles of health data processing including detailed coding and basic abstracting. The incumbent will be responsible for, but not limited to, the following duties:

• On-Unit Health Record Technician Coordinator & Chart Runner: (1) the ability to physically travel to/from and perform duties on all treatment units in the secured area of the hospital; (2) Perform on-unit documentation review, chart maintenance, thinning, and/or filing; (3) Regularly walk long distances; (4) Routinely carry/transport medical record binders (estimated 10-15 lbs). Conduct accurate, structured, concurrent, quantitative audits within the secured area of the facility.
• Abstracting/Coding: Assign ICD-10 codes in accordance with ICD-10-CM guidelines. Abstract data from a variety of sources.
• Patient Access to Medical Record Appointments: When assigned, attend and participate in appointments for Patient Access to Medical Record. Assist the HIMD Coordinator with release of information. Reply to correspondence related to requests from Social Security Administration and Department of State Hospitals-Sacramento.
• Discharge/Court Leave Record Processing: Analyze medical records for deficiencies when a patient is discharged or goes to court. Communicate incomplete record findings to responsible staff and take steps to have the record completed.

• Other duties as assigned.

2. BE TRANSFER ELIGIBLE: Have current employment with the State of California in the same classification or in a classification that is comparable (employees who wish to be considered based on transfer eligibility must meet the minimum qualifications of the classification per DPA Rule 250), OR
3. BE REINSTATEMENT ELIGIBLE: Have obtained previous permanent employment with the State of California in the same classification or in a classification that is comparable.

MINIMUM QUALIFICATIONS :
Education: Equivalent to completion of the twelfth grade. (Additional qualifying experience may be substituted for the required education on a year-for-year basis.) AND Either I: One year of California state service clerical experience which includes training and supervised work experience in the procurement and processing of health data OR II: Two years of clerical experience in the medical or health records section of the health facility. (Successful completion of an academic curriculum in medical records science in an accredited school may be substituted for the required general experience on a year-for-year basis.)
