Job Description

Purpose of Position:

The Emergency Pharmacist is responsible for providing comprehensive clinical pharmacy services for the ED and all associated areas (e.g., pediatrics, trauma, urgent care), including pharmacokinetic and therapeutic consultations. In addition to performing the duties at a more difficult and responsible level, the following are typical tasks performed.

DUTIES & RESPONSIBILITIES:

Ability to work independently with minimal direct supervision. Exercises sound professional judgment when managing the medication therapy of a group or unit of patients as part of an interdisciplinary team and participating in rounds. Rotating between tours and shifts; performing the following tasks:
1. Be familiar with the standards of patient care; understand the medical conditions, treatment goals, treatment drugs, and protocols for Ambulatory Care Sensitive Conditions (ACSC: Asthma, COPD, Diabetes, CHF, Hypertension, Pneumonia, Dehydration, and UTIs) and other patients that are referred.
2. Interview (meet) the patient during the ED visit. Conduct a medication therapy review and communicate with the medical and/or nursing staff as needed in order to provide comprehensive patient care. This includes but is not limited to:

a) Counsel all patients referred by the Emergency Department.

b) Conduct a medication reconciliation comparing medications the patient was on prior to the ED visit and incorporating any changes in regimen or instructions and communicate the same to the patient/caregiver. c) Develop a plan for resolving medication-related problems.

d) Communicate appropriate information and clinical concerns/interventions to the patient's physician. e) Follow-up phone calls to the patient, patient's caregiver, and/or pharmacy and/or to confirm compliance with filling prescriptions/compliance with medication regimen and understanding of instructions.
3. Prepare written notes (document) consistently on all patients seen. Enter progress notes (documentation) in the electronic medical record. Enter a note in the computer at the conclusion of the patient encounter.
4. Work collaboratively and help coordinate activities with the medical team to develop the patient discharge plan, promote adherence to the treatment plan, and encourage primary care provider (PCP) follow-up for optimal medical management. Participate in pharmacy and ED PI projects.
5. Participate in debriefings and/or weekly meetings with the Emergency Department and/or pharmacy leadership.
6. Provide support during emergency situations as needed by the ED team.
7. Compound IV medications as needed.
8. Recommend dose adjustments when required based on patient renal, liver function and other lab results.
9. Monitor document and report adverse drug reactions (ADRs).
10. Check for patient drug allergies and update all computer information.
11. Verify the quality of the physician's medication reconciliation at discharge. Compare the written discharge prescriptions against the patient's home medication list, the current medication list, and the discharge summary. Communicate clinical concerns/interventions with the patient's physician and resolve prior to discharge. Escalate to the attending or physician, physician advisor as needed.

Minimum Qualifications

For Level I:
1. Possession of a valid NY State license to practice pharmacy.

For Level II: In addition to meeting the qualification requirements of the lower level:
1. Two years of post licensure experience; or
2. Completion of a post graduate specialty residency or fellowship program; or
3. Current Pharmacy Board Certification.

For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:
1. One year of post licensure supervisory experience or specialty clinical experience.
