Harlem Hospital Center has been a bedrock of the Harlem community since 1887. The hospital provides a wide range of medical, surgical, diagnostic, therapeutic and family support services to the residents of Central Harlem, West Harlem, Washington Heights and Inwood. Harlem is the largest hospital in the area, capable of treating the most seriously ill. Job Description

Under direction, with moderate latitude for the exercise of independent initiative and judgment, provides professional pharmacy services in a hospital, clinic or other health care setting. The following are typical tasks performed at Assignment Level I:

SUMMARY OF DUTIES AND RESPONSIBILITIES:
• Fills prescriptions and patient orders, resolves discrepancies in patient therapy, and follows-up on problems.
• Utilizes clinical data in evaluating physician orders to ensure appropriate therapy.
• Provides and documents drug information and patient counseling.
• Documents interventions, adverse drug reactions (ADRs), and medication errors.
• Directs and utilizes personnel including technicians, aides and clerical staff.
• Assists in the monthly inspection of the patient care units and the Pharmacy.
• Maintains adequate medication supply in individual work area.
• Participates in Pharmacy staff in-services and meetings relating to Systemwide and/or health care setting programs.
• Operates and troubleshoots automated dispensing systems, as needed.
• Advises prescribers on alternatives for non-formulary drugs.
• Utilizes Outpatient and Inpatient computerized systems to enter and access patient care information.
• Coordinates and analyzes statistics and/or clinical data.
• Participates in quality management and utilization review programs.
• Additional duties assigned.

Minimum Qualifications

Possession of a valid NY State license to practice pharmacy.
