Description

The Hematology Oncology Nurse Navigator is a professional, clinical Registered Nurse responsible for guiding and supporting oncology patients throughout their continuum of care. This role emphasizes coordination of care, continuity of care, patient education, and providing emotional support to patients and their families. The Hematology Oncology Nurse Navigator ensures that patients receive comprehensive care following the nursing process and adhering to the standards of professional nursing practice.

Key Responsibilities:

  • Facilitate seamless coordination of care across various healthcare settings, ensuring continuity of care for oncology patients from diagnosis through treatment, and into survivorship or end-of-life care.
  • Provide ongoing support and education to patients and their families, helping them navigate the healthcare system and understand their treatment options. Serve as a primary point of contact for patients, addressing concerns and guiding them through their healthcare journey.
  • Apply clinical knowledge and expertise in oncology to advocate for the best patient outcomes. Engage in patient advocacy to ensure that patients receive appropriate care in a timely manner, reflecting evidence-based practices.
  • Stay current with advancements in oncology care by participating in ongoing education, training, and professional development. Provide education to patients, families, and healthcare staff as needed.
  • Engage in ethical decision-making and utilize evidence-based practices in patient care. Contribute to the development and implementation of policies and procedures that promote quality and safety in oncology care.
  • Work collaboratively with multidisciplinary teams, including physicians, social workers, and other healthcare professionals, to develop and implement comprehensive care plans. Take on leadership roles within the care team to advocate for best practices in patient care.
  • Participate in quality improvement initiatives aimed at enhancing patient outcomes and ensuring compliance with professional standards and regulations.


Salary Range: $54.07 - $123.04 hourly
