Job Description

SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Fills prescriptions and patient orders, resolves discrepancies in patient therapy, and follows up on problems.
  • Enforces policies that address medication use throughout the institution.
  • Compounds, manipulates and dispenses sterile and non-sterile medication, including hazardous medication, according to USP 795, 797 and/or 800 guidelines.
  • Orders, receives and distributes controlled substances. This includes identifying and resolving discrepancies.
  • Escalates serious or emergent matters that require action by a supervisor or director.
  • Notifies pharmacy informatics and/or contacts the appropriate vendor or helpdesk to address information technology failures.
  • Utilizes clinical data in evaluating physician orders to ensure appropriate therapy.
  • Provides and documents drug information and patient counseling.
  • Documents interventions, adverse drug reactions (ADRs), and medication errors.
  • Directs and utilizes personnel including pharmacy technicians and aides. This includes verification of the completion and quality of all assignments.
  • Performs monthly inspection of the patient care units and the Pharmacy.
  • Maintains adequate medication supply in individual work area.
  • Arranges the procurement of medication from other institutions when needed on an emergency basis.
  • Participates in Pharmacy staff in-services and hospital-wide programs and meetings.
  • Operates and troubleshoots automated dispensing systems, and repackaging equipment, as needed.
  • Advises prescribers on alternatives for non-formulary drugs. In the absence of the director's timely approval, the pharmacist may approve the use of nonformulary medication provided all other aspects of the policy are observed and the medication is already in stock.
  • Utilizes Outpatient and Inpatient computerized systems to enter and access patient care information.
  • Coordinates and analyzes statistics and/or clinical data.
  • Participates in quality management and utilization review programs.
  • May coordinate the work of a small group to accomplish a project or assignment.
  • May participate in the preparation and review of policies and procedures.
  • May assist in the development, implementation and execution of Pharmacy staff training curriculum.
  • In the absence of the supervisor, may assist in the supervision of a Pharmacy unit or function. This includes arranging of unscheduled coverage for a subsequent tour.
  • Complies with KCHC and departmental polices for safety, quality improvement and infection control.
  • Provide patient/family education appropriate to age.
  • Adheres to organizational policy for annual in-service education and health assessment.
  • Maintains confidentiality of patient and hospital records.
  • Able to lift 45 pounds and stand, walk, bend the entire shift and or working hours.


Minimum Qualifications

Possession of a valid NY State license to practice pharmacy.
