Harlem Hospital Center has been a bedrock of the Harlem community since 1887. The hospital provides a wide range of medical, surgical, diagnostic, therapeutic and family support services to the residents of Central Harlem, West Harlem, Washington Heights and Inwood. Duties & Responsibilities
Under direction, with moderate latitude for the exercise of independent initiative and judgment, supervises a Pharmacy unit and/or provides professional pharmacy services in a hospital, clinic or other health care setting. In addition to performing the duties of Level I at a more difficult and responsible level, the following are typical and additional tasks performed at Health System Pharmacist for Procurement Level I.
1. Proficient in computerized order entry systems (Inpatient & Outpatient). Have basic knowledge of automated dispensing system (PYXIS).
2. Operate and troubleshoot automated dispensing system (PYXIS), as needed.
3. Utilize inpatient computerized system to enter and access patient care information.
4. Review prescriptions/orders, obtain clarification as necessary and regularly consult with medical staff, nursing personnel and others in order to coordinate delivery of high-quality pharmaceutical services to patients.
5. Review the prescriptions/orders for appropriateness, accuracy and assure all necessary information is present (e.g., age, dose, allergy, route, frequency, drug- drug, food- drug, drug- lab interaction, etc.,)
6. Fills patient's orders, resolves discrepancies in patient therapy and follow-up on problems.
7. Utilizes clinical data in evaluating physician orders to ensure appropriate therapy.
8. Document interventions, adverse drug reactions (ADRs) and medication errors.
9. Resolve questions regarding formulary and non-formulary medications and advice health care providers on alternatives for shortage/outages.
10. Assist in replenishing drug bin and shelves and keeping the pharmacy in a neat and orderly fashion.
11. Direct and utilize personnel including technician's aides, and clerical staffs.
12. Maintain adequate medication supply in the pharmacy area and inform supervisor of impending shortages.
13. Fill patient's unit dose medication cassettes, prepares parenteral and oral medications using sterile aseptic technique.
14. Assist in daily/monthly inspection of patient care units & pharmacy (e.g., fridge temp log, cleanliness of environment, medication expiration checks etc.) and emergency cart preparation and checks (as assigned)
15. Oversee & direct the activities of pharmacy technicians/volunteers/students to ensure appropriate pharmaceutical care. In the absence of the supervisor, may assist in the supervision of a Pharmacy unit or function based on seniority.
16. Handover and communicate unresolved issues with supervisor and the subsequent tours.
17. Participate in pharmacy daily huddles, staff meetings, in- services and other meetings as delegated.
18. Ensure compliance with regulatory agencies, hospital and department rules, regulations and policies.
19. May participate in quality management and utilization review programs.
20. Maintain an acceptable attendance record and adheres to Hospital's dress code.
21. May participate in the preparation and review of policies and procedures as needed
22. May coordinate the work of a small group to accomplish a projects or assignments
23. May assist in the development, implementation and execution of pharmacy staff training curriculum.
24. In the absence of the supervisor, may assist in the supervision of a pharmacy unit or function.
25. Keep abreast in pharmaceutical knowledge by reading and attending workshops, conferences and C.E. programs
26. Handover and communicate unsolved issues with supervisor and the following with subsequent Tour.

For Assignment to Level I
1. Possession of a valid NY State license to practice pharmacy.
