Summary:
The Infusion Patient Care Navigator is responsible for assisting nurses to provide outreach efforts to patients needing care coordination for infusion treatments. This role will utilize the established model of care management. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Required Job Qualifications:
• High school diploma/GED required.
• Previous experience in an ambulatory setting required.
• One year experience using electronic medical records.
• Strong written and verbal communication skills.
• Proficient in the Microsoft Office Suite.
• Exceptional interpersonal skills.
• Strong ability to multi-task in a large centralized scheduling center.
• Ability to function independently, to organize, and be self-motivated.
Preferred Job Qualifications:
• Associates or Bachelor's degree.
• Bilingual highly desirable
Responsibilities:
• Provides outreach for all patients due for follow up and new patient appointments according to policy and procedures.
• Monitors compliance with scheduled appointments for procedures, tests and consults.
• Documents all correspondence in electronic medical records as necessary.
• Works to improve identified outcomes (i.e. high utilizer patients) by using quality measures.
• Works with care team to identify extrinsic barriers to adherence (i.e. transportation, financial concerns or pharmacy) and work with patient and team to alleviate barriers.
• Daily monitoring of Epic pools for pending approved treatments awaiting scheduling.
• Acts as a liaison to multiple departments, agencies and provider offices.
• Collects time of service payments, posts payments in Epic
• Greets patients at the front desk, checks patients in and out of infusion and performs practice designated activities to support new patients.
• Works cooperatively in a team environment.
• Demonstrates compliance with RUMG Customer Service and Performance Standards.
• Answer Epic in-basket messages and secure chats timely and efficiently.
• Schedules, cancels and reschedules patient treatments.
• Other duties as assigned.
