Position Summary:
The Oncology Care Specialist partners with patients, families, the multidisciplinary team, and community resources to provide well-coordinated, timely, compassionate, exemplary, multidisciplinary care throughout the continuum in an organized and time conscious manner. The Oncology Care Specialist also facilitates timely access to appropriate healthcare and resources for patients and their families all while maintaining active communication among all medical staff, interdepartmental staff, external customers, etc. to ensure barriers to care are removed. The Oncology Care Specialist is also responsible for coordinating and developing the lung screening program in conjunction with the Department Director.

Essential Functions:
  • Coordinates the care and experience of our Oncology patients through the cancer care continuum. Schedules appointments with health care specialists and support services for the Oncology patient Assists patients in accessing cancer care and navigating healthcare systems Works to improve the timeliness from diagnosis to treatment for patients by identifying organizational barriers to care Uses organization, time management, problem-solving, and critical thinking to assist patients efficiently and effectively Initiates efficiently securing appointments for patients in an effort to decrease patient outmigration/increase patient retention Sets up appointments with the financial counselors Builds trust by being accessible, accurate, supportive, and acting within scope of practice Performs administrative duties accurately and efficiently Supports a smooth transition of patients across screening, diagnosis, and into active treatment, survivorship, and/or end-of-life care, working with the patient's clinical care team Arranges transportation as needed for patients
  • Interacts with staff, physicians, patients and families, and internal and external departments/offices to ensure customer satisfaction. Communicates with ancillary departments to define and resolve specific problem areas and ensure continuity in patient care Demonstrates ability to work effectively with others and promote good interpersonal rapport among others Demonstrates win/win attitude & problem solving skills Seeks input from physicians regarding satisfaction with level of patient care and/or quality concerns Demonstrates support for departmental and institutional leadership Participates actively in Oncology departmental meetings and committees to promote the Oncology Program and actively seek feedback for improvement Participates in interprofessional teams to provide patient- and population-centered care that is safe, timely, efficient, effective, and equitable Participates in outreach and marketing to employed and independent providers to ensure they are familiar with the services provided Maintains confidential information
  • Coordinates and leads the development of the lung screening program Evaluates all lung screening orders for eligibility criteria Analyzes the EPIC workqueue with lung screening orders on a daily basis Communicates with patients to determine if they meet the eligibility criteria for a lung screening Tracks all lung screening patients in electronic navigation software in a consistent, organized manner Ensures all monthly lung screening reminder letters are sent to patients Communicates effectively with referring providers/their offices regarding patient eligibility for a lung screening Monitors lung screening patients to ensure compliance with follow-up recommendations from lung screening Reviews, processes, and records follow-up recommendations from lung screenings Coordinates any follow-up recommendations for further diagnostic workup with the ordering provider's office
