THIS IS A GRANT-FUNDED POSITION

JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: The Intervention/Outreach Specialist assists youth and families to connect with supportive services through screening and referrals in the Center for Youth Advocacy and Well-Being (CYAW). This position assists children and families in coordinating the delivery of health, wellness, and mental health services by assisting with the navigation of child-serving, educational and human/social services systems.

DUTIES AND RESPONSIBILITIES:

  1. Interviews youth and families to determine options for securing and maintaining resources to support needs and aids in the navigation of community-based services.
  2. Advocates for and empowers youth and families to seek the best solution for their youth and families in education, medical, and behavioral health systems.
  3. Aids youth and families to access treatment and resources necessary to build resiliency and foster success in the home, school, and community; meets with youth and families in the community or school.
  4. Assists families with creating plans to help youth and families overcome potential barriers.
  5. Assists youth and families to achieve maximum benefits of program services.
  6. Facilitates referrals to appropriate resources and community programs.
  7. Provides educational sessions and training to families, community partners, and UTHSC staff.
  8. Maintain communication with community agencies and resource providers within his/her assigned region.
  9. Completes referrals and inputs case notes in the case management system.
  10. Participates in community events and trainings.
  11. Provides follow-up to the youths, families, and agencies. Completes reports and assists leadership in fulfilling the obligations and outcome tracking of the program.
  12. Performs other related duties as assigned.


Qualifications:

MINIMUM REQUIREMENTS:

EDUCATION: Bachelor's Degree in Social Services, Criminal Justice, or related field. (TRANSCRIPT REQUIRED)

EXPERIENCE: Two (2) years of experience working with children and family; OR Master's Degree in lieu of experience; OR a combination of education and experience to equal six (6) years. Must have a valid Driver's License and vehicle insurance.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of resources in the community to assist youth and families get connected with providers in the child and family serving systems.
  • Knowledge and experience in fostering community partnerships and collaborations.
  • Skills working with youth and families who have behavioral health concerns.
  • Ability to relate to youth and families.
  • Ability to maintain confidential information.
  • Ability to be self-motivated, maintain a friendly demeanor and exhibit tact and professionalism both in person and on the phone.
  • Knowledge and understanding of the system of care values and principles.
  • Demonstrated experience collaborating with diverse stakeholders.
