The Early Childhood Supports & Services program, located in Jefferson Parish, will serve as the local early adopter site for children (0-5) and their families in Jefferson, Orleans, St. Bernard, and Plaquemines Parishes. The LMSW role is to provide comprehensive case management support to clients within the ECSS network. They will be skilled at developing collaborative relationships with clients and community partners. The LMSW will identify and coordinate care plans across ECSS and community systems and connect clients with available resources. The LMSW is responsible for maintaining client records and staying informed about developments within ECSS, the ECSS Network, and community systems. The LMSW will report to the Site Director of Early Childhood Supports and Services (ECSS).• Ability to identify and leverage client's strengths to address barriers and assist them in implementing strategies to achieve client outcomes.
• Ability to coordinate cross-functional initiatives to facilitate collaborative partnerships and enhance outreach efforts.
• Ability to coordinate assessments, services, and support strategies to achieve client's defined personal outcomes.
• Ability to facilitate informed decisions through varied experiences, ongoing support, and creative choices to improve client outcomes.

• Ability to establish and maintain a primary relationship with recipients for effective service provision.
• Ability to ensure case management accessibility for clients, their families, and associated parties at convenient times and settings.
• Ability to collaborate with community providers to resource their services and advocate on client's behalf.
• Ability to maximize client's self-determination, cultural sensitivity, and potential for growth and change.

• Successful completion of a Master's level graduate degree in a mental health field (e.g., social work, counseling).
• Licensed or license-eligible mental health professional in Louisiana.

• Bilingual communication skills.
• Experience with young children.

• Ability to communicate in a culturally sensitive and trauma informed manner with all families.

• Ability to engage and communicate successfully with ECSS team members to best advocate and coordinate services for families.
