Summary
• Under the direction of the department leadership provides nursing care for assigned patients. Performs care, treatments, and procedures within scope of practice and established guidelines and provides a safe and therapeutic environment.
Responsibilities
• Obtains information for assigned patients during bedside report, or hand off communication to/from another department. • Implements patient care instructions per physician orders. • Consults with and keeps Charge Registered Nurse informed of activities, symptoms, reactions, and changes in the patient's condition with regard to assigned patients. • Administers medication within scope of practice and established policies. Maintain the MAR (Medication Administration Record). Observes and documents patient reactions to medications to determine efficacy/side effects. • Provides prescribed education to patient and family members related to health, treatments and medications. • Participates in data collection related to patient status and needs. Documents all aspects of patient care. • Provides appropriate pain management techniques and instructs the patient and family regarding pain management. • Performs patient hygiene duties as needed. • Completes and documents a patient assessment each shift • Performs nursing procedures and patient care per policy and procedure. • Assists with admitting, discharging and transferring of patients; assessments of newly admitted patients to be completed by a Registered Nurse. LPNs receiving a patient from a lower or same level of care and who have already had an assessment completed and documented by a Registered Nurse during the previous 24 hours, do not need an additional co-signed assessment by a Registered Nurse. Patients transferring to a higher level of care will require an assessment by a Registered Nurse. • Assists with maintenance of unit safety. • Assists with training/orienting new staff. • Performs other related duties as assigned.
Qualifications
• Required o Licensed Practical Nurse licensure in the State of Nevada. o American Heart Association (AHA) Basic Life Support (BLS) / Cardiopulmonary Resuscitation (CPR) for healthcare providers. o Basic computer skills for: Accessing and navigating CTRH intranet for forms, policies, and procedures. Accessing and entering data on organization wide software such as timekeeping system and hospital email. Accessing and using internet and specific applications required to perform job duties. o National Institutes of Health Stroke Scale (NIHSS) certification at or within 90 days of hire. o NVCI/CPI Training (Non-Violent Crisis Intervention) initial certification within six (6) months of hire, refresher completed annually. o IV Competency completion within 6 months of hire. Preferred o Three (3) years of LPN experience in an acute care hospital. o Obtain RN license within 24 months of hire
