  • Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status.
  • Functions in accordance with the professional standards of the Ohio board of nursing.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
  • Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
  • Monday-Friday with rotating weekends.
  • This position is in Home Care Services.
  • This position will cover the Elyria Area.
  • 100% travel to the patients homes to provide them with care.


    Qualifications:
    • Other Accredited Program Completion of accredited practical nurse program or currently enrolled in a nursing program.
    • 2+ years Nursing Experience (Level II only) required.
    • Knowledge of LPN scope of practice according to the Ohio board of nursing.
    • Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (ie. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices).
    • Level II Only: 18 CNE's per year. • 3 CNE's in clinical specialty • At least 5 CNE's in Leadership • Other CNE requirements will be agreed upon with manager.
    • Level II Only: • 3 Peer Reviews. • Committee Involvement. • 5 criteria with evidence of each.
    • Knowledge of LPN scope of practice according to the Ohio board of nursing required.
    • Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (ie. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices).
    • Licensed Practical Nurse (LPN) in the State of Ohio required.
    • Basic Life Support (BLS).
    • IV certification (unit depending).
