Summary:
The Manager RN, Care Management has specialized knowledge, education, and experience in the fields of chronic disease management, disease education, and nursing. The Manager is responsible for program development and operational oversight of key programs in the department. The manager works with a multidisciplinary team that includes both inpatient and outpatient teams to facilitate effective care management, coordination of services at the appropriate level of care, and implement sustainable transition plans. Responsibilities:
• Responsible for managing staff with complex patient caseloads.
• Provides chronic disease management for patients and creates patient-centered care plans. Educates patients on the levels of healthcare, entitlements, and community resources.
• Advocates for patient/family/caregiver needs and facilitates referrals for continuity of care.
• Adheres to evidence-based treatment guidelines in facilitating care for chronic disease management, promotes self-care management, and coordinates transitions of care and DME requests.
• Implements optimal care coordination that enhances patient and population adherence to treatment plan while also assessing for any acute needs.
• Provides leadership, support, and clinical expertise within Care Management teams to achieve desired outcomes.
• Assists in program development and process improvement.
• Functions as a role model within the team and is attuned to the cultural needs and health care disparity issues that may be inhibiting patients/caregivers' engagement in their healthcare.
• Manages complex cases that are escalated to management in order to provide proactive care planning, coordinated transition plans, and implements readmission avoidance strategies.
• Provides leadership and facilitates communication between the ambulatory and inpatient settings to ensure effective and sustainable care transitions from hospital to home and care plans within community care settings.
• Supports team education and training functions related to chronic conditions and transitions in care coordination.
• Coordinates interdisciplinary conferences, serves on Ambulatory Nursing and Population Health committees and leads work groups to address care management issues.
• Maintains relationships with referring Rush clinics through consistent and timely communication.
• Contributes to program development and best practice standards.
• Develops and leads implementation of department goals, change initiatives, and population management initiatives.
• Develops educational strategies that promote patient, family and/or patient population learning and self-management.
• Models and maintains a quality-based, patient-centered approach to achieve department and institutional goals and process improvements.
• Participates in research to evaluate project initiatives. Applies evidenced based practice.
• Synthesizes and applies available data and information to initiate action relevant to identified populations.

Required Job Qualifications:
• Master's degree in Nursing from an accredited university required.
• Current State of Illinois licensure as a Registered Nurse.
• At least five years of clinical nursing experience in public health or care management.
• Excellent communication, interpersonal, and team building skills.
• Process improvement skills; ability to perform tasks independently, prioritize workload, problem solve, and analyze data.
• Proficient in the Microsoft Office Suite.
• Demonstrates effective problem-solving skills in a healthcare setting, preferably an academic medical center.
• Demonstrates the ability to address barriers that are inhibiting a patient's adherence to managing medical recommendations needed to achieve good health outcomes.
• Demonstrates accountability for implementing, instructing and acting as a resource on policies related to patient-centered care management
Preferred Job Qualifications:
• Certification in care management, care transitions, or another nationally recognized area preferred.
