Job Title: MDS Coordinator

Job Summary

Works in collaboration with the Nurse Coordinators to ensure accurate, timely completion of Minimum Data Set and Resident Assessment Protocols.

Essential Job Functions

  • Completes a Minimum Data Set (MDS) on assigned residents/patients per RAI Manual.
  • Ensures MDS assessments and care plans are done accurately and promptly.
  • Informs the interdisciplinary team promptly of assessment deadlines and findings relevant to their area of responsibility.
  • Conducts MDS and care plan audits on all assigned residents.
  • Ensures that significant changes are identified through the MDS process and are communicated with the other disciplines.
  • Collaborates with interdisciplinary teams (nurses, therapists, social workers, etc.) to create an individualized care plan.
  • Evaluates residents to ensure they meet the Medicare payment criteria, including clinical eligibility and availability of Medicare days.
  • Stay updated on federal and state regulations related to MDS assessments, ensuring compliance, and implementing any necessary changes to maintain adherence to these guidelines.
  • Provide guidance, education, and training to staff members involved in the assessment process to ensure accurate and consistent data collection.
  • Look for potential risks and changes that may affect reimbursement.
  • Accurately know the resident and skilled documentation necessary for maximum reimbursement
  • Educate nursing staff on necessary documentation for each resident.
  • Work closely with the HIM department to identify any errors in a diagnosis and ensure that queries are made to the Physician.
  • Attend and participate in mandatory meetings, in-services, and training.
  • Performs audit to determine MDS accuracy issues related to Quality Measures


Job Requirements

Education:

  • Graduate from an accredited school of nursing, BSN preferred


Experience:

  • One year of experience required in Minimum Data Set (MDS)


Licenses/Certifications:

  • Current RN license by the Florida State Board of Nursing
  • BLS preferred
  • ACLS preferred
  • Certified Registered Nurse Assessment Coordinator preferred


Abilities Required

  • Knowledge of nursing care practices for rehabilitation, geriatric and dementia patient populations
  • Demonstrates a caring attitude toward MJHS residents, patients, family members, employees, and other facility guests
  • Knowledge of OBRA & PPS MDS scheduling and completion
  • Excellent computer skills
