JOB SUMMARY:

UT Southwestern Medical Center is offering a new opportunity for a full-time LCSW Social Worker in the Cystic Fibrosis Clinic. This role involves providing coverage for the Cystic Fibrosis Program.
  • Shift: 8-hour days.
  • Work From Home: Hybrid schedule 2-3 days in office and the rest working remotely.
Your duties may include but is not limited to the following:
  • Provide brief therapy to Cystic Fibrosis patients, and mental health coordination.
  • Provide medical social work to patients with Cystic Fibrosis.
  • Psychosocial assessments
  • Half of the duties dedicated to therapy. The other half dedicated to medical social work.

EXPERIENCE | EDUCATION:
  • Master's degree in Social Sork
  • And two (2) years' experience is required.
JOB DUTIES:
  • Assists in development and implementation of Case Management. Provides expertise in the area of social work as it pertains to management of patients.
  • Assists in coordination of client care through formulation of appropriate plan of care via accurate assessment of patient's social, financial and insurance situation.
  • Develops short- and long-term plans to facilitate patient care and discharge.
  • Utilizes crisis intervention and problem-solving skills to facilitate and enhance patient and family's ability to meet acute and ongoing medical and psychosocial needs.
  • Networks with community agencies to assure appropriate client access.
  • Maintains documentation and meets department and Zale Lipshy Uni- University Hospital documentation standards.
  • Identifies cases with ethical and legal considerations and makes referrals. Confers with supervisor.
  • Communicates with other health care providers, patients, families, other hospital personnel, and visitors. Demonstrates effective communication with adolescents, adults, and older adults.
  • Maintains system for tracking placement of patients after discharge on monthly basis.
  • Has knowledge of Department and Unit Performance Improvement programs and is aware of expected participation. Identifies the need for professional growth and seeks development opportunities.
  • Provides education to staff, patients, and community resource affiliates.
  • Performs other duties as assigned.
