• Work location: Texas Health Ft. Worth, 1301 Pennsylvania Avenue, Ft. Worth, TX 76104

• Kidney Transplant department

• Work hours: Full-time, 40 hours/week, Monday-Thursday 8:00AM-5:30PM and Fridays 8:00AM-12:00PM Noon.

Here's What You Need

• Master's Degree Social Work Required

• 2 Years of healthcare related Social Work experience Preferred.

• Experience with Kidney/Renal Dialysis/Transplant Strongly Preferred

• LMSW - Licensed Master Social Worker Upon Hire Required Or
LCSW - Licensed Clinical Social Worker Upon Hire Required

• CPI - Crisis Prevention Intervention - Maintained Annually - within 90 days Required.

What You Will Do

• Fundamental knowledge and skills in Social Work assessment and intervention as well as alternative levels of care, community resources and healthcare industry.

• Sound interpersonal skills and able to multitask and think critically in a fast-paced environment.

• Independently practices as part of the healthcare team.

• Transplant evaluations; Pre-Op and Post-Op and Living Donor evaluations.

• One-on-One with patients and inpatient rounding for new transplants

• Collaborate presentations to multi-disciplinary teams.
