Job Description
Responsible for coordinating the individualized care of patients throughout the continuum of care services, eliminating barriers to healthcare and enhancing patient and physician satisfaction. Serve as a member of the multidisciplinary team functioning as patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services. Provides support to patients and caregivers through guidance, direction and liaising in outpatient settings. Collaborates with physicians, nurses and staff to assess and provide educational needs of palliative patients and their families across the continuum.

Qualifications:

• Current CT RN license required.
• Bachelor's Degree in Nursing (BSN) required.
• 5 years nursing experience, 2 years patient/staff education experience required.
• Working with Geriatric population preferred.
