Summary

MaineHealth Care At Home (MHCAH), is seeking a Nurse Practitioner to join our Palliative Care-Hospice Group. The Palliative Care team provides quality, cost-effective medical care for patients at Sussman Hospice House, Rockport, Maine.

The Nurse Practitioner (NP) works autonomously but in collaboration with physicians to deliver primary medical care and intraepisodic care for hospice patients. The NP provides advanced practice nursing services through management of the terminal condition and developing treatment plans for patients at home and in the hospice house to meet the hospice mission to "Provide holistic, compassionate, quality comfort care that enhances the lives of our patients and their families while respecting the right to self-determination".

Minimum Qualifications:
    • Advanced knowledge of nursing theory and practice, and additional specialized medical training, in order to provide expanded nursing practice at a level generally acquired through completion of a Master's Degree.
    • Current license and registration to practice as a Nurse Practitioner as required in the State of Maine.
    • Need 2 years' experience in Hospice and or Palliative Care- advanced certification in Hospice & Palliative Nursing preferred.
    • Palliative Medicine specialty certification and/or palliative practice experience equivalent/ eligible for specialty certification.
    • Knowledge of health care management and the hospital care environment.
    • Ability to work independently, effective time management and problem-solving skills.
    • Interpersonal skills and communication skills to engage as a specialist with a variety of personnel (e.g., patients, employees, physicians, and other health care professionals.)
