Summary

The Director manages patient care services for the Lee Health Hospital at Home division (H@H). Actively participates in organizational leadership functions and collaborations with other organizational leaders and medical staff in designing and delivering acute patient care services in the home setting.

Reporting to the System Director, H@H the Nursing Director will provide administrative management of H@H division development and ongoing operations. The Director will be responsible for ensuring that operational and clinical components required for H@H service delivery align with local, state, and federal regulatory policy. Supports clinical and operational staff in the delivery of clinical care in the home, asset management and logistics, contracting, quality, safety, pharmacy and all ancillary services and capabilities, remote patient monitoring, and virtual care. Responsible for driving services that result in high levels of workforce, patient, and caregiver experience.

The Director must demonstrate a deep understanding of hospital-based care and the ability translate that knowledge and experience to establish hospital-level care in a home setting. In home care is inclusive of clinical, operational, regulatory, and financial leadership. The Director will collaborate with the H@H dyad to execute the H@H operational model at launch, during program evolution, and through continuous optimization of the Lee Health H@H care delivery operations. This will include the identification of patients who can receive care through the H@H Division to improve access to care, maximize facility capacity, and deliver cost effective care while maintaining service and quality on the same level (or higher) of a patient in a traditional hospital-based inpatient setting. Responsible for directing and coordinating patient care, within H@H, through established standards subscribed by the American Nurses Association and in accordance with the Florida Nurse Practice Act.

Requirements

Education: Masters degree in nursing or a Bachelor of Science in Nursing combined with a masters degree in business Administration, Healthcare Administration, or related field.

Experience: 4 years of experience in a Nursing Leadership role in the acute care setting. Hospital at Home experience preferred. Nursing or APP background required, 4+ years in healthcare leadership, experience with cross-functional teams and projects, experience building or leading highly matrixed teams, knowledge and experience managing contracts.

License:Registered Nurse Licensed in the State of Florida or Licensure requirement met within 60 days of hire for non-Florida residents.
