Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City's efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support.

Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient's clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management.

The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. Position Overview

The OTxHU Reentry Services Supervisor is responsible for overseeing the day to day operations of the OTxHU reentry social workers providing individualized discharge plans and reentry support to all patients residing in the OTxHU. These services include but are not limited to linkages to benefits, housing, medical & mental health care, nursing home/skilled nursing placement, employment, and family/community supports. Working with Reentry and Transitional Services leadership the OTxHU Reentry Services Supervisor will have the opportunity to help develop and lead a new, holistic discharge panning model. This position will require strong collaboration with leadership across all CHS departments and external partners (e.g., the NYC Department of Correction, public defense agencies, community-based organizations).

Responsibilities Include:

Develop and implement workflows and documentation for delivering pre-release support and discharge planning services to all patients in the OTxHU.

Supervise social workers providing direct services to patients; provide back-up coverage as needed.

Support the creation of comprehensive clinically driven discharge plans.

Assist social workers in identifying and utilizing community resources for medical and mental health care, substance use treatment, entitlement benefits, and housing.

Lead in- service training of staff and evaluate all aspects of social workers' work performance, including participating in counseling and disciplinary processes when necessary.

Promote and support collaboration with the inter-disciplinary treatment team and other key stakeholders.

Liaise and collaborate with court services.

Conduct quality assurance reviews to ensure the provision of clinically appropriate tailored care, and, when applicable, compliance with the Brad H stipulation.

Generate reports and reconcile data related to patient needs and service delivery.

Participate in quality improvement and policy creation efforts to expand and institutionalize best practices in care and discharge planning services for these and other vulnerable populations.

Other responsibilities as assigned by supervisor or leadership.

Departmental Preferences:
  • Valid license as an LMSW issued by the NYSED, with strong preference for LCSW
  • At least two years supervisory experience in a health care setting; particularly with medically or psychiatrically complex patients
  • At least two years' experience providing direct services to justice-involved patients or in a hospital setting
  • Strong clinical skills (written, clinical interviewing, counseling/therapy)
  • Strong professional boundaries
  • Experience using electronic medical record systems
  • Comfort with data reporting, including experience with Excel
  • Strong interpersonal skills and excellent communication skills (written and verbal)
  • Knowledge of benefit entitlements, community-based resources for the previously incarcerated population
  • Familiarity with jail-based discharge planning services and workflows
  • Strong organizational/time management skills
  • Ability to work independently; dependable and able to work under pressure
  • Ability to effectively present information and respond to questions
  • Work experience in a correctional or secured setting
  • Bilingual in Spanish


Minimum Qualifications
1. Master's Degree in Public Administration, Public Health, Health Care Specialization, Social Work, Psychology or related field; and
2. Three (3) years of increasingly responsible experience in hospital or regulatory agency administration, with particular emphasis on development and evaluation of mental health delivery services; or
3. Possession of a valid license and current registration to practice in a mental health field issued by the New York State Education Department (NYSED).
