Job Description

Position Description:

CHS is seeking a Post Release Services Coordinator (Community Liaison Worker III) to support the Post Release Services unit and other projects under the Office of Reentry and Transition Services (RATS). The Post Release Services unit aims to ensure timely and client-centered connection to health care and support services for those currently incarcerated and recently released from NYC jails. Services include, but are not limited to: PORT Line, a general helpline that assists with navigating health care and service landscape in jail and community; Office of Health Insurance Services (OHIS), which supports efforts to ensure Medicaid coverage upon release for eligible patients; facilitation of medication provision for patients released to ATI programs; contract management and close coordination with various community partners.

The Post Release Services Coordinator will work closely with CHS discharge planning teams and community programs to ensure medication provision to patients entering alternative to incarceration programs. Additionally, will support various contract management activities.

This position requires excellent organizational and communication skills, resourcefulness, responsiveness, health literacy, and ability to manage tasks independently. Ideal candidates will be highly self-motivated and familiar with the health care service landscape in NYC jails, especially processes related to discharge planning.

Detailed Tasks:
• Coordinate with multiple CHS teams to ensure timely community scripts and walking medication delivery to 55 Water St.; utilize electronic health record for prescription requests, process documentation, and medical record provision
• Coordinate Rikers clearance processes for contracted community vendor staff
• Work closely with community program and contracted vendor staff
• Track and support various contract management initiatives and other projects within Post Release Services unit, as directed.

Minimum Qualifications
1. Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or,
2. Education and/or experience equivalent to (1 above). Study at an accredited college in Sociology, Psychology or other behavioral sciences may be substituted on a year-for-year basis for the experience described in (1 above). However, all persons must have at least one year of full-time experience as described in (1 above).
