Job Description Summary

Under the direction of the Clinical Director of the VNA, oversees the tele-monitoring program including daily monitoring and triage of patient data, patient and staff education, marketing, and equipment care and set up.

ESSENTIAL FUNCTIONS

1. Oversees daily operations of the tele-monitoring program.
a. Manages patient transmissions of data.

b. Follows up alerts by contacting patients.

c. Assesses patient status via phone and provides instructions as appropriate.

d. Notifies physician and other team members of patient status as appropriate.

e. Responsible for assigning installation/removal of units in patient's home.

f. Ensures that all consent forms and physician orders for program are completed.

g. Collects pertinent data from PTCT for use as Benchmarking

h. Assures orientation and education for clinicians covering telemonitoring alerts

i. Maintains knowledge of technology in use for telemonitoring

2. Educates patient, families, and staff about the tele-monitoring equipment and processes.
a. Orients new staff to tele-monitoring equipment and processes.

b. Instructs patients and families about use and care of equipment.

c. Provides clinical direction from the perspective of process and outcomes.

d. Functions as the resource person to staff, Physicians and community on tele-monitoring.

e. Partners with the marketing team to introduce opportunities for tele-monitoring

f. Accepts opportunities to create program awareness in serving communities

3. Completes and submits documentation within 24 hours of all patient interactions as well as physician orders as applicable.
a. Utilize contact note in PTCT for documentation of all MD /patient contacts

4. Accountable for the management and growth of the tele-monitoring program.
a. Ensures that policies and procedures are implemented correctly and safely.

b. Ensures that staff is able to safely and effectively educate patients and utilize data.

c. Reviews all new referrals prior to installation of the units for appropriateness.

d. Ensures that equipment is installed in a timely manner.

e. Participates in outreach activities to market the program internally and externally.

f. Develops policies and procedures specific to tele-monitoring program.

g. Develop/upgrade educational materials as needed

h. Works with Home Health Aide Manager to assist in training of Home Health Aides for equipment pick up.

5. Promotes collaboration with other members of healthcare team.
a. Initiates and participates in case conferences.

b. Assists staff with integrating tele-monitoring into electronic documentation/plan of care.

c. Attends staff meetings of all teams at least 2x a year

d. Maintains a current power point presentation for potential customers

6. Maintains integrity of equipment.
a. Keeps accurate records of location of all equipment.

b. Works with materials management/manufacturer t to ensures equipment is maintained and in working order.

c. Ensures that equipment is returned when patient is discharged in a timely manner.

d. Makes recommendations for purchase of new equipment/ trading of equipment as growth ensues

e. Identifies and maintains a professional relationship with software representative

f. Participates in software user group

7. Maintains and updates knowledge and skills as evidenced by attendance logs and CEU certificates.
a. Attends and participates in management team meetings as well as division/SSH committees.

b. Attends in-services both within and outside the organization.

c. Works toward showcasing Home care division work with telemonitoring on a local and regional level

8. Is responsible for Division's operational excellence: assures delivery of quality services in accordance with the Home Care Division policies, procedures and professional standards.
a. Manages all program activities so that quality services are provided in an efficient and effective manner.

b. Services provided meet all applicable regulatory requirements.

c. Participates in an effective quality improvement program as evidenced by reports with outcome nurse.

d. Maintains program productivity standards; monitors status and reports as requested.

e. Supervises department activities in a manner consistent with organization-wide programs and procedures.

f. Participates in the development of Divisional policies and procedures that reflect the organization's standards.

g. Participates in JCAHO and any other regulatory surveys

9. Is responsible for the fiscal management of program: assures proper utilization of the Division's financial resources.
a. Develops program budget within designated deadlines according to all budgetary policies and procedures.

b. Effectively utilizes resources within established budget: notifies Director of all variances anticipated, or actual.

c. Adheres to procedures regarding acquisition of, and payment for, goods and services, as evidenced by appropriate and timely processing of requisitions and invoices.

d. Recommends way to reduce expenditures and enhance revenues without compromising quality of services.

e. Provides annual accomplishments document every October.

10. Effectively communicates Divisional, organizational and industry information to staff.
a. Attends organization informational meetings as required/requested.

b. Gathers, interprets, and delivers information to staff in a timely fashion.

c. Assures staff understands information, as evidenced by staff compliance with and participation in organization programs, seminars, training and related activities.

d. Communication with staff is effective, as observed by director.

e. Uses hospital communications resources to inform internal and external audiences of divisional news/achievement/policies.

11. Embraces technological solutions to work processes and practices.
a. Demonstrates clinical proficiency and competency in the automated clinical record as evidenced by successful completion of education program.

b. Utilizes phone system and telecommunication services as instructed; this includes phone, e-mail, paging systems, faxing and cell phones.

c. Oversees/Develops in partnership with education manager, staff education related to the appropriate usage of tele-monitoring equipment.

d. Utilizes and is proficient on departmental or role specific software as assigned.

e. Demonstrates a base knowledge and utilization of the Microsoft suite of office products: word, excel, power point.

12. Fosters a "Culture of Safety" through personal ownership and commitment to a safe environment.
a. Understands individual roles/responsibilities during emergency situations in a patient's home or at the office, i.e. Emergency Preparedness.

b. Educates staff to and ensures adherence to patient identification policies and procedures.

c. Uses personal protective equipment appropriately.

d. Adheres to respiratory etiquette guidelines.

e. Handles hazardous waste according to departmental protocol.

f. Utilizes proper body mechanics when performing all aspects of the job.

JOB REQUIREMENTS

Minimum Education - Preferred

Bachelor of Science degree in nursing from an NLN approved school of nursing.

Minimum Work Experience

Two years of acute care experience including and two years of community health experience preferred.

Required Licenses / Registrations

RN - Registered Nurse

Required additional Knowledge and Abilities

Knowledge of cardiac disease processes required.
