Job Summary

Under the direction of the Assistant Director, Community Health Worker Collective, Jefferson Collaborative for Health Equity, the Program Manager for the Community Health Worker (CHW) Collective coordinates the care delivery workflow for Jefferson CHWs and provides supervision, coaching, and professional development for Jefferson CHWs. A key responsibility is to provide training and support to participants in the CHW Academy and related activities, including shadowing at worksites and input to the training curriculum.

Essential Functions
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Provides supervision and management of staff
  • Works with leadership to implement strategy and planning related to the Jefferson Collaborative for Health Equity's programming
  • Develops and strengthens on-going relationships with internal and external stakeholders and works with community leaders and social service agencies to identify community needs and resources
  • Collaborates with internal and external stakeholders to improve the quality of program delivery
  • Maintains communication with internal and external stakeholders
  • Works on special projects and performs other duties as assigned

PROGRAM SPECIFIC FUNCTIONS
  • Using a trauma-informed lens and strengths-based approach, provides ongoing supervision/coaching sessions for CHWs and identifies and builds on the CHW's strengths and assets
  • Trains CHWs on performance outcomes and coaches CHWs to meet identified outcome goals
  • Documents supervision/coaching session notes
  • Keeps Assistant Director apprised of performance, interpersonal, or other programmatic issues and assists with addressing to resolution
  • Provides classroom facilitation and professional development sessions to CHWs and participants in the CHW Academy on core CHW-related topics
  • Assists Lead CHWs in the management of the Jefferson CHW Community of Practice and related activities, including scheduling meetings, developing meeting agendas, identifying training topics, and distributing communications
  • Ensures community resource information is communicated to CHWs and updated in the Jefferson Health Community Resource repository (FindHelp)
  • Assists with educating Jefferson Health about the Community Health Worker Collective

Qualifications

Education:

Bachelor's degree in human services, social work, public health, psychology, or related field required. Master's degree preferred.

AND

Experience:

Minimum of 3 years' experience in program management, supervision, and/or coaching role preferably in health care or human services setting. Experience in community health work a plus.

Conditions of Employment

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
