Job Description

Under the direct supervision of a Registered Professional Nurse, the Psychiatric Social Health Technician (PSHT) performs therapeutic and other mental health related activity, maintains direct contact with people who have social and emotional problems and handicaps, and interests with clients in daily activities of living, and insures a properly maintained physical, social and clinical environment.

RESPONSIBILITIES:
  1. Responsible for the delivery of safe, quality patient care and services under the direction of a Registered Professional Nurse in accordance with Bellevue Hospital's policies and procedures
  2. Member of a designated care team and collaborates with team members in achieving goals
  3. Receives assignment from the Registered Nurse
  4. Accepts aspects of care delivery for individual patient/family as delegated by the Registered Nurse
  5. Introduces self to patient/family and explains what will be done for/with each patient
  6. Identifies what the patient/family and explains what will be done for/with each patient
  7. Gathers patient data and documents on flow sheet:
    1. Temperature, Pulse, Respiration
    2. BP
    3. Height, Weight
    4. Finger stick
  8. Reports observation of patients/data to the RN consistently throughout the tours
  9. Assures patient privacy at all time
    1. Draws curtain/closes door while providing care
    2. Assures appropriate dress
    3. Does not discuss patient in public areas
  10. Reports patient experience of pain/discomfort immediately to the RN
  11. Provides patient care as delegated by RN
    1. Bathing
    2. Grooming/shaving
    3. Ambulation
    4. Turning and positioning
    5. Skin Care
    6. Passive and active range of motion
    7. Toileting
    8. Assist with transportation
    9. Changes bed linen
    10. Prepares patient meals and assist with menu selection
    11. Delivers/removes trays, distributes water, nourishment, ice, etc.
    12. Feeds patient
    13. Mouth care
    14. Foot care
    15. Nail care
    16. Simple dressing
    17. Applies hot and cold packs
    18. Assists RN to perform EKG's
    19. Collects specimen for urine, and labels accurately
    20. Assists with moving patients and assures belonging sent with them
    21. Assists nurses and MD's performing procedures
    22. Ambulates patient to bathroom or assists to commode: does not use diapers
  12. Prepares patient and/or examination room for test/procedure:
    1. Cleans and sets up instruments and trays
    2. Position patient on table, bed, stretcher, wheelchair
  13. Assists with orienting new support staff team members as instructed by RN/Clinical Instructor
  14. Documents patient data/care provided and signs name and title legibly
    1. Assessment form (demographic data, vital signs)
    2. Daily Patient Assessment and Plan Review Sheet
    3. Multidisciplinary Patient/Significant Other education sheet
    4. Progress Note
    5. Intake and Output
    6. Restraint flow sheet
    7. 1:1 Special Observation sheet
  15. Thoroughly completes Environment of Care/Safety and other required documents and forwards those to Head nurse/Charge Nurse/RN In-Charge
    1. Code Red
  16. Demonstrates understanding of human growth and development relative to age
  17. Understands cultural differences of patients/families and accommodates them as directed by the RN
  18. Removes equipment not in use from patient's room and returns them to appropriate department for cleaning/repair
  19. Accompanies patient to community agencies and facilities
  20. Performs all other duties assigned by Supervisor and/or designee.


Minimum Qualifications

Experience assisting patients with daily living and therapeutic activities and performing other related functions in a hospital or a health care setting; and
2. Holds or obtains a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA) within sixty (60) calendar days of appointment to the title.
