PRIMARY FUNCTION :

Monitors cardiology patients for outpatient services with direct oversight by patient's provider.

ESSENTIAL FUNCTIONS:
  • Collaborates with physicians and assists in the coordination and follow-through of the cardiac clinical plan as documented.
  • Triages phone calls to physicians, utilizing clinical nursing skills to assess the degree of response.
  • Completes medication prior authorizations
  • Completes Genetic testing prior authorization and ordering.
  • Handles patient emergencies both in office and from home.
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Utilizes advance skills in rhythm interpretation.
  • Demonstrate knowledge for programming Pacemakers and Internal Cardiac Defibrillators.
  • Demonstrate general knowledge for testing Implantable Loop Recorders.
  • Demonstrates knowledge of Electrophysiology procedures including but not limited to Ablations, Cardioversion, EP Studies and implantation of PPM & ICD.

OTHER FUNCTIONS AND COMPETENCIES:
  • Assures the completion of all necessary forms including disability, homecare, hospice, medical clearance and Pre procedure device evaluation forms.
  • Facilitates the direct admission of patients when necessary, both planned and emergent.
  • Acts as a liaison for all pharmaceutical and device reps, homecare and hospice agencies.
  • Enrolls patients in Cardiac Rehab programs as appropriate.
  • Oversees transitions of care from the inpatient to the outpatient setting with the participation of the multidisciplinary team.
  • Responds to online patient messages through EPIC.
  • Triage walk in patients into the office.
  • Cover anti-coagulation program on an as need basis.
  • Initiate medication refills to physicians for authorization
  • Maintain BLS and ACLS


Qualifications

EDUCATIONAL/TRAINING REQUIREMENTS:

RN, graduate of an accredited nursing program.

BSN required

CERTIFICATES, LICENSES, AND REGISTRATION:

Licensed Registered Nurse in the Commonwealth of PA

ACLS or BLS preferred

EXPERIENCE REQUIREMENTS :

2 years of experience with Cardiology patients strongly preferred. Background working with a cardiac physician practice, a cardiovascular lab or at least 2 years hospital nursing experience is highly desirable. Experience working with critical care services strongly preferred.
