Brigham Health Harbor Medical Associates is a large multispecialty Ambulatory Practice with multiple locations across the South Shore. We believe that patient care comes first! Our team of physicians, nurse practitioners, physician assistants, nurses and support staff recognize the importance of prompt, expert and compassionate care for patients whose needs span general health maintenance to treatment for complex medical problems. Our physicians, who are on the staff at South Shore Hospital as well as Brigham and Women's Hospital, provide advanced care for patients with a broad range of medical conditions.

Schedule/Location:

We are currently hiring a full-time, 40-hour RN Care Coordinator to support our patients at mainly two of our practices:

  • 56 New Driftway, Suite 1010, Scituate, MA 02066
  • 6 Shipyard Drive, Suite 2A, Hingham, MA 02045


As a hybrid opportunity, the position typically covers 1 day a week on-site but does require flexibility to go on-site more when needed (days can be flexible dependent on candidate's schedule) and remote coverage 4 day per week.

In addition, our team does meet regularly at our South Weymouth office on-site for trainings, staff meetings and team gatherings.

Due to the hybrid nature of the role, we ensure that our employees receive required technology and training to be proficient and independently productive in all job responsibilities regardless of work location. Employees are responsible for designating a workspace within the remote work location that is private, safe, ergonomic, and free from distractions for all hours worked.

The Opportunity:

Working closely with high-risk patients, the Ambulatory RN Care Coordinator is responsible for establishing, implementing, monitoring, and evaluating high-quality cost-effective care plans. Our coordinator collaborates with practice Physicians and Program leadership, strategizing about optimal and efficient patient care plans and approaches. This position requires a broad knowledge of clinical care and systems' management, case management expertise, prudent nursing judgment, health care re-imbursement, sound problem solving skills, independent thinking, excellent organizational and interpersonal skills, creativity, flexibility, and the ability to multi-task.

Primary Responsibilities to include but not limited to:

  • Consults with patients' physicians and/or Program Medical Director about high priority patients and potential care management plans.
  • Initiates telephone or in person contact with eligible patients to perform initial assessment, healthcare education, and develop a realistic care management plan.
  • Incorporates knowledge of case management, levels of care, and utilization management principles to implement high-quality cost-effective care plans.
  • Performs program specific patient outreach, education, and recruitment.
  • Influences utilization of health care resources by providing direct care coordination to patients, encouraging enrollment in disease and case management programs, providing care coordination intervention and follow-up prior to and after interaction with health care system, e.g., inpatient, ED visit, outpatient services, etc.
  • Establishes a consistent communication and reporting schedule for periodic contact with providers and patients to review patient status and progress toward goals.
  • Attends and presents case reviews at practice meetings, program meetings, and care coordination meetings.


Qualifications

Qualifications

Required:

  • MA Registered Nurse
  • Three to five years of acute care experience in a hospital setting.


Preferred:

  • BSN/MSN.
  • Ambulatory experience working in a health center or physician's office.
  • Experience working in a post-acute setting such as L-TAC, rehab, skilled nursing facility, or homecare.
  • Managed Care experience.
