Hours/Location

This hybrid opportunity offers a unique work-life balance. After 4-6 weeks of training, you will provide onsite coverage for four days per week and remote coverage for one day per week. Our RN Care Manager will work at one to two of our sites in Salem, MA, alongside another Care Manager. However, the training will take place in any of our North Shore sites.

To ensure your success in the role, we will provide you with the necessary technology and training regardless of your work location. We prioritize your well-being and productivity, and encourage you to designate a private, safe, ergonomic workspace within your remote work location.

The RN Care Manager will contribute significantly to the enhancement of patient care services along the continuum of care. By adopting a patient-centered approach, you will collaborate with primary care and specialty physicians, practice staff, acute care and community-based providers, patients, and their caregivers to develop, implement, monitor, and evaluate high-quality, cost-effective care plans. Your dedication to improving the patient experience, involving patients in self-care, and optimizing healthcare resource utilization will have a profound impact on our patients' lives.

Primary Responsibilities to include:

  • Perform comprehensive nursing and psychosocial assessments and outreach on a designated patient population. Many of our patients have multiple chronic diseases.
  • Develop patient-centered care plans in conjunction with the patients' PCPs, multidisciplinary team members, caregivers, and patients.
  • Monitor patients' progress, identify barriers to self-management, and assist with navigation and advocacy.
  • Collaborate with primary care and specialty providers to identify high priority patients for care management.
  • Conduct certain care management activities, such as telephonic contacts post-discharge and referrals to community-based services.
  • Monitor patients' progress and plan of care, identifying and reporting issues and system barriers.


Qualifications

Required:

  • Registered Nurse with a current, valid license to practice in the state of Massachusetts.
  • Minimum 5 years nursing experience.
  • Completion of an accredited nursing degree program.
  • Proven experience in using technology proficiently to enhance productivity and efficiency in daily tasks.


Preferred:

  • Bachelor's degree preferred.
  • Certification in case management or other applicable professional certification, such as CCM or ACM.
  • NCQA requirements experience or knowledge helpful.
