What You'll Do:
  • Assist with maintaining accurate records on all referral sources, status of existing or pending referrals, admissions, discharges, and hospitalized patients as needed.
  • Ensure that a primary liaison is maintained between the company and referral sources.
  • Involve other community agencies in the referral process as needed.
  • Compile patient information from referral sources including specific patient demographics, insurance and/or payor sources to ensure quality of communications, information and care for patients.
  • Assist with ensuring patients meet the face-to-face regulations.
  • Provide skilled nursing care to patients in accordance with Physician's orders in compliance with Federal, state and company regulations.
  • On-call rotations as needed.
  • Request additional information for referrals that are incomplete and document requests in the clinical record.
  • Complete clinical record review/audits
  • Manage and document phone calls from physicians, clinicians, patients, referral sources and communicate patient updates/new orders to clinicians.
  • Follow up on labs and other clinical diagnostics test, physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow up on needed plan of care modifications; communicate such to clinicians.
  • Review clinicians visit notes to ensure timely, complete, appropriate, and accurate submission of documentation by visiting staff.
  • Review, evaluate, and supervise service delivery to ensure appropriateness of care and utilization of services, equipment and supplies through activities such as medical records review and case conferences.
  • Other duties as needed.

Qualifications:
  • Current RN license with at least three (3) years of experience in a clinical setting
  • Previous Experience in home care and home health. Knowledge of managed home healthcare is preferred
  • Candidate must possess excellent verbal and written skills
  • Solid computer proficiency in Microsoft Office including Excel and web based application
  • Committed to exceptional customer service and meeting customer needs
  • Ability to complete tasks in a multi-phased, fast-paced environment
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
  • Complies and maintains current CPR certification
