Job Summary:
The Children's Healthy Eating and Active Living Center (C-HEAL) Care Coordinator ensures the overall success of the C-HEAL clinic by collaboratively working with patients, physicians, Medical assistant, pediatric surgeon, pediatric psychologist, child life team, adult metabolic bariatric surgery center and LCSW, to integrate the key features of the multidisciplinary team and provide optimal care for our patients.

The Care Coordinator will also promote education and self-management skills; assist in identification of appropriate providers, facilities, and community resources in an effort to improve or maintain patients' social, emotional, functional, and physical health status; development of patient registries as available; provide education and guidance for patients undergoing bariatric surgery; and help direct the insurance approval process for our bariatric surgery patients. The focus of the Care Coordinator is to promote whole person health support.

This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Directs the practice's quality improvement process under the guidance of the C-HEAL medical director.

Education:
Required:
Degree in Exercise Science, Nutrition Science, nursing or related health degree, or a licensed practical nurse certification.

Required:
Exceptional skills of independence, organization, communication, problem-solving, professional interactions, and human relation skills, as well as analytical skills and problem solving ability. Proficient with processes to build team and participate in cross-functional teams. Ability to work within specified timeframe requirements. Position requires Motivational Interviewing techniques and Adult Learning Styles. Excellent oral and written communication skills with problem solving abilities. Exceptional interpersonal communication skills are required. Experience with data collection, documentation review, and statistical analysis.

Preferred:
2 years' experience in Utilization Management, Case Management or Care/Disease Management. Extensive knowledge HCC scores and Risk Stratification.

Department Position Summary:
The C-HEAL Care Coordinator contributes to Erlanger Health System through support of the philosophy and objectives.

RESPONSIBILITIES:
C-HEAL Care Management
Work with the C-HEAL clinical teams as a resource on care management for all patients

Work with the dietitian to review all patients prior to their upcoming visits, identifying which patients require visits from the exercise specialist and/or the dietitian and whether labs are required, and communicating with the medical director and team members as necessary

Promotes collaborative teamwork within the department as well as the Care managers team; able to work with all peers in a team situation.

Directs the quality initiative projects for the clinic under the supervision of the C-HEAL medical director.

Purchasing incentives for C-HEAL patients, maintaining goal sheets and patient resources

Overseeing food insecurity project including food vouchers

Scheduling pediatric residents, UT nurse practitioner students, UTC exercise science students to rotate within the C-HEAL center

Must be able to identify resources and navigate patients to the appropriate resource outside the care managers scope of practice.

Performs additional tasks requested within the scope of Care Mangers role.
