Seeking Registered Nurses for an RN Care Manager position

Location: remote but must be local to NYC (could be asked up to 6x a year in the office and that could increase, must be flexible)

Experience:

• highly prefer someone with a background and familiarity with patients with HIV diagnoses
• case management background
• Fluent in Spanish preferable
• HIV, Transgender care or Mainstream/traditional Medicaid preferable

Caseload: 250 but could be more. Will have a CMSC partner and every month will swap outreach calls to half the membership unless something clinical comes up.

This is strictly Medicaid care management but will work with other Medicare plans for coordination of benefits.

Pay: $48-52/hr DOE
