Your Role: As an RN Care Manager, you'll work in our bustling Internal Medicine office in Dover, NH, collaborating with an interdisciplinary healthcare team. Your primary responsibility is to manage and coordinate patient-centered care for individuals with chronic conditions, helping them achieve their health goals through personalized care plans and coaching.
What You'll Do:
- Patient-Centered Care: Conduct comprehensive health assessments and develop individualized care plans in partnership with patients, caregivers, and healthcare providers.
- Proactive Care Coordination: Manage care transitions and collaborate with hospitals, specialists, and community resources to ensure seamless care.
- Health Coaching: Empower patients to take control of their health through coaching, focusing on self-management and shared decision-making.
- Collaborative Teamwork: Be a leader in organizing the care team around patient needs, and work closely with your colleagues to solve challenges and improve patient outcomes.
- Hybrid Work Schedule: After successful training, enjoy a 4-day on-site and 1-day remote work schedule.

Qualifications
What We're Looking For:
- Bachelor's Degree in Nursing (or equivalent experience) and an active RN license in NH.
- 3+ years of nursing experience in inpatient or outpatient settings.
- Strong clinical assessment, health coaching, and patient education skills.
- Proficiency in medical standards of care and clinical nursing skills.
- Ability to work both independently and as part of a team.
- Experience with health information technology and electronic medical records.
- Certification in Chronic Care Management is a plus!
