Nurse Case Manager (RN) - Hiring Now in Atlanta

THIS IS NOT A REMOTE POSITION

We are looking for a dedicated and compassionate Registered Nurse (RN) Case Manager to join our Complete Care at Home team. As an RN Case Manager, you will play a crucial role in coordinating comprehensive home care for our Geriatric clients, creating custom care plans, and supervising Caregivers. This position offers the opportunity to work closely with clients, families, and caregivers to ensure the highest quality of care in the comfort of their homes. This position is 50% in office and 50% on the road traveling to visit current clients and performing starts of care for new clients.

Schedule: Monday - Friday with on-call availability

As a supervisor, some of your duties include (but are not limited to):

Responsibilities:
  • Conduct initial client assessments and develop individualized care plans.
  • Coordinate and oversee all aspects of client care.
  • Provide health education, and emotional support to clients and their families
  • Monitor client progress and update care plans as needed
  • Monitor Caregiver's performance and evaluate at 90 days and annually
  • Educate clients and caregivers on disease processes, treatment options, and self-care techniques.
  • Advocate for client's needs and collaborate with healthcare providers to ensure continuity of care.
  • Document client care and maintain accurate medical records in compliance with agency policies and regulatory requirements.
  • Participate in case conferences, team meetings, and in-service education sessions as required.
  • Be available for on-call work in emergency situations - infrequent


Qualifications:
  • Current RN license in GA (required)
  • Nursing Degree, BSN preferred
  • Minimum 5 years of clinical nursing experience ( required )
  • Minimum of 2 years of Clinical Manager experience, preferably in home care or Home Health care or a related setting ( required )
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Reliable transportation and willingness to travel within the metro Atlanta Area
  • BLS certification.
  • Proficiency in electronic health records (EHR) systems
