Medix is currently looking for an experienced RN Case Manager to take on a role located in New York, New York! This position is fully onsite for assisted living!

The RN Case Manager implements and enhances strategies for exceptional resident and family experiences. They foster a service-oriented culture and collaborate with internal and external stakeholders to improve service excellence. This includes implementing best practices, educational programs, and patient-centered initiatives. Additionally, they serve as the subject matter expert for case management and service excellence, utilizing data to ensure responsiveness and alignment facility-wide.

Responsibilities:
  • Greets and transitions new residents smoothly into the facility.
  • Orientates residents to daily routines, addressing adjustment concerns.
  • Manages admission documentation and addresses resident concerns.
  • Leads continuous improvement of resident experience.
  • Oversees satisfaction surveys and implements improvement programs.
  • Collaborates on employee orientation to uphold the organization's values.
  • Develops facility-wide service and educational programs.
  • Coaches leadership on organizational values and excellence.
  • Coordinates residents' medical, psychosocial, and service needs.
  • Assists residents with personal shopping and maintaining ties.
  • Resolves complaints and grievances in a patient-centered manner.
  • Networks to stay updated on industry trends and maintains facility strategies.
  • Monitors data for responsiveness and alignment.
  • Produces reporting for external partners.
  • Designs and implements clinical and service programs.
  • Evaluates resident needs and program capabilities.
  • Arranges services from public and private sources.
  • Facilitates alternative living arrangements and discharge plans.
  • Maintains regular communication with responsible parties.
  • Monitors hospitalized residents and facilitates re-admissions.
  • Participates in Management Meetings and informs staff of resident information.
  • Documents all Case Management activities for compliance.

Requirements:
  • Experience working with MLTC healthcare plans, or case managment, skilled nursing
  • Case management background preferred
  • NY State Registered Nursing License
