Department Description:

CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.

Job Summary:

The CHA PACE RN provides nursing care to frail and disabled 55+ adults as a member of a PACE (Program of All-inclusive Care for the Elderly) interdisciplinary team (IDT) with the goal of supporting frail and disabled older adults so that they can live in the community with support. Nurses are expected to perform with a high degree of independence and personal accountability. They must be proficient in the geriatric assessment of elders with chronic illness, dementia, and mental illness as well as perform assessments, triage in the home or clinic setting to support and/or direct the participant's plan of care. This is an onsite clinic nurse/triage position that includes MDS-HC completion for the program.

Qualifications:
  • Current RN MA licensure required
  • ARC or AHA BLS Healthcare Provider certification required
  • Phlebotomy skills preferred
  • B.S. in nursing preferred
  • Reliable transportation to make daily home visits in the CHA PACE service area required
  • 1-3 years of home care experience preferred
  • Highly developed geriatric assessment and clinical skills
  • Experience at a PACE plan, working on an interdisciplinary team, etc. is desired
  • Fluency in Spanish, Haitian Creole, Portuguese, etc. is desired
  • Clinical experience with geriatric, behavioral health and/or disabled populations is desired
