Position Summary:
An active member of the interdisciplinary healthcare team the Case Manager facilitates internal and external delivery of healthcare services. The Case Manager integrates the roles of Utilization Management, Case Management, and Discharge Planning. The Case Manager is responsible for care coordination and development of the discharge plan through collaboration with the healthcare team and internal/external resources. The job objective is to promote a collaborative interdisciplinary care delivery model to achieve optimal clinical and financial outcomes with adherence to regulations and compliance.
Essential Functions
1. Case Manager independently performs initial and ongoing discharge screens/evaluations to determine general health behaviors, practices and resources that will influence the patient and family's abilities to manage chronic conditions optimally.
2. Collaborate with the health care team to determine the extent of functional deficits and areas for potential improvement.
3. Assess emotional status and quality of relationships and adequacy of social support systems, identifying new sources of support.
4. Prioritizes immediate plan for barriers to discharge including medication availability for use, shelter and appropriate food, and primary care physician coordinating care resources accordingly
5. Assures patient /family/caregiver understands and has access to all prescribed post-discharge therapies.
6. Communicate with patients/families admission to discharge
7. The Case Manager independently performs clinical reviews based on the Utilization Review Plan with follow-up actions identified for medical necessity and level of care
8. Maintains clinical and financial documentation
9. Participates in performance improvement processes as well as other duties as required by the job

Required Licenses
Registered Nurse
- Current/active North Carolina or compact RN license
- BLS optional
