The Harris Center for Mental Health and IDD ("The Harris Center") is on the hunt for a dedicated Service Coordinator who is passionate about empowering individuals with intellectual disabilities. In this vital role, you'll help our clients navigate various services-from medical and psychiatric assistance to residential and financial support, and beyond. Your mission is to unlock the full potential of person-directed planning processes, shaping meaningful outcomes according to each individual's unique needs and aspirations. You'll serve as a critical bridge between consumers, legally authorized representatives, program providers, and other key collaborators.

Your Role in Action:
  • Conduct comprehensive assessments, adhering strictly to our department's policy and procedural guidelines.
  • Ensure client welfare through regular, at least quarterly, monitoring tailored to each consumer's unique needs.
  • Maintain the highest standard of data integrity by updating consumer files as necessary.
  • Be responsible for timely documentation, ensuring all records are updated within our established timelines.
  • Uphold a safe and healthy environment by complying with all safety and health requirements and staying updated with mandatory training programs.

What qualifications you will need:

Education
  • Bachelor's degree (B.A.) from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
  • OR any Bachelor's degree with one (1) year experience working with persons with intellectual and developmental disabilities

Experience
  • Minimum of one year experience in the human services field preferred.
  • Experience working with persons with intellectual disabilities preferred.
  • Must possess effective written, verbal communication, and organizational skills.
  • Must be able to work autonomously in a fast paced environment.
  • Effectively communicates and works with consumers, family members, visitors and fellow employees in a manner that demonstrates knowledge of cultural diversities.

Other Key Qualifications
  • Must maintain a valid Texas Driver's License and be insurable under Agency policy.
  • Must have reliable transportation.
