Social Services Coordinator

Our skilled nursing facility is looking for a full-time Social Services Coordinator.

The primary purpose of the Social Services Coordinator is to identify and provide for each resident's social, emotional, and psychological needs, to develop the resident's full potential during his/her stay at the facility. The Social Services Coordinator should posses leadership qualities needed to guide individuals of varying backgrounds toward a common goal for the betterment of a patient's health and quality of life.

Qualifications and Skills

• Bachelor's degree in Social Work or related field is preferred
• Clinical experience in a skilled nursing facility setting preferred.
• Familiarity with post-acute care, rehabilitation hospitals, sub-acute rehabilitation facilities, and the local healthcare market preferred.
