JOB SUMMARY: The Director of Social Services is responsible for planning, organizing, coordinating, directing, and reporting on all aspects of the social service department. Implementation of group and one-to-one psycho-social programming and involvement in continuity and quality of care. Will meet with patients, family members, ancillaries, and interdisciplinary team members regarding care planning.

JOB REQUIREMENTS:

- Must be physically and mentally capable of performing routine job duties.

- An understanding of the psycho-social dynamic of the geriatric population

- Ability to empathize, communicate, and provide guidance to residents, family members, and staff

- Maintain knowledge of ODH, federal, and state regulations

- Must be caring and compassionate

MAIN DUTIES:

- Support the facility's philosophy of care and strive to achieve its goals and objectives

- Review Psycho-Social care plans for resident appropriateness

- Review monthly progress notes, quarterly assessments, and social histories

- Identify any change in status or condition and communicate with families and other discipline staff regarding any needed change in goals

- Coordinate monthly family meetings and special programs for residents, families, and the community.

- Coordinate all aspects of hospice care for those residents or families who choose hospice care.

- Coordinate discharge planning for appropriate residents.

- Perform other related duties as directed by the Administrator.

QUALIFICATIONS:

- A master's or bachelor's degree in social services or related field preferred.

- Two years of experience in the field of social work, preferably in a long-term care environment.
