• As part of the admission process, meets with residents and/or responsible parties to review policies on advanced directives, living wills, DNR status, and resident rights and discharge planning. Implement documents as appropriate with residents and/or their responsible parties.

• Conduct and document social service evaluations including identification of resident needs/issues and arrange for appropriate social support services.
• Counsel prospective residents and/or their responsible parties as needed for adequate preparation for admission to the facility.
• Provide assistance to residents and/or their responsible parties to ensure a satisfactory adjustment to life in the facility.
• Address difficulties with emotional adjustment to the facility through interviews, counseling, and referrals when indicated.
• Provide assistance to residents and/or their responsible parties in obtaining financial services, as appropriate.

• Meet with new residents and/or their responsible parties to answer questions and ensure they have been informed of their personal and property rights.

Minimum Qualifications:

  • • Bachelor Degree in Social Work required, Master's Degree in Social Work preferred.
    • Current license LSWA, LGSW, LCSW, LCSW-C in the State of Pennsylvania preferred.
    
    
    
    • Minimum of Bachelor's Degree in Behavioral Health.
    • One (1) year supervised social work experience working directly with individuals in a long term care or sub-acute health care setting.
