Position Details/Preferred Qualifications
This position is for a social work float who assists staff at Rainbow Babies & Children's, Macdonald Women's, and the Rainbow Center for Women & Children in Midtown, who have high workload or coverage needs. The role is essential in order to ensure timely and safe discharge. Will provide psychosocial assessments, supportive counseling, and crisis intervention. Previous experience working with Pediatrics or OB is strongly preferred. Will see patients that are dealing with a variety a psychosocial issues, such as substance abuse, domestic violence, adoption, homelessness, teen pregnancy, poverty, positive toxicology screens, referral to the County, mental health issues, etc. Will provide psycho-social assessments, supportive counseling, crisis intervention, and discharge planning. The weekly hours are 17 ½ hours. Required days are Mon, Fri, AND 1 weekend day: either every Saturday OR every other Saturday or Sunday (**could also be half days on Saturdays).

What You Will Do
- Provides psychosocial assessment / intervention to assist patients/families (including clinically complex)
• Provides age specific care to patients/families in all age groups. - Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.
• Develops plans with attention to individual patient and family goals
• Provides linkage to community resources and UH network services to facilitate optimal care - Communicates/collaborates with the work team using a multidisciplinary approach.
- Provides guidance/ consultation to peers/team regarding routine psychosocial issues and /or discharge planning issues.
- Provides supervision to social work interns if LISW eligible (completion of supervision hours).
Additional Responsibilities
- Performs other duties as assigned.
- Complies with all policies and standards.
- For specific duties and responsibilities, refer to documentation provided by the department during orientation.
- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Qualifications:
Education
- Master's Degree from an accredited school of Social Work (Required)
Work Experience
- Hospital/Health Agency experience. (Preferred)
Licenses and Certifications
- Licensed Social Worker (LSW) (Required)
- Licensed Independent Social Worker (LISW) within 1 Year (Preferred)
