Social Worker - Care Coordination - FT - Days

Description
Responds to consults in a timely fashion.
Effective and efficient communication to physician and patient.
Knowledgeable of community agencies resources and able to facilitate processes involved in obtaining the same.
Able to provide patients/families with information regarding available financial assistance options, i.e. Medicare, Medicaid, etc.
Facilitation of referrals for financial assistance by providing names and numbers of contact persons and locations.
Acts as a patient/family advocate.
Provides emotional support to patients/families in a crisis situation or those with high risk diagnosis.
Knowledgeable of available support groups in the area and able to supply information as needed.
Provides concise and informative documentation in the medical record regarding patient, family or physician encounters, services or resources provided or information given regarding these.
Has working knowledge of state and federal requirements and is able to counsel patients/families regarding available benefits.
Assists with the development of care plans for patients identified through case management.
Assists with meeting age specific psychological needs of patients identified by the team.
Assists with transfer arrangements for case management cases.
Provides consultation on difficult cases and initiates care conferences when necessary.
Provides input into resource utilization and appropriate care settings.
Maintains positive working relations with skilled care, long term care, rehab, nursing home facilities as well as home health and durable medical agencies.
Qualifications
Minimum Education
Bachelor's degree in Social Work, required.
Minimum Work Experience
One (1) year of experience in related field required. Hospital and/or Long Term Care experience preferred.

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
