SUMMARY:

We are currently seeking a Social Worker I to join our team. This full time role will work days. This position will be working to link newly diagnosed and out of care individuals to care. This is a grant funded role, currently funded through August 2025 with potential for extension.

Purpose of the position: Under general supervision, and in collaboration with the health care team, this position evaluates individualized social treatment plans requiring the application of social work theory and methods in the prevention, treatment, or resolution of psychosocial problems

RESPONSIBILITIES:

  • Assesses patients to determine individual needs and establish treatment/service goals
  • Develops case plans providing ongoing psychosocial assessments through interviews with patients and families
  • Makes referrals to appropriate community resources and arrange for services
  • Follows through on treatment/service plans
  • Identifies patient situations that require intensified service and bring them to the attention of the supervisor for help or referral
  • Participates on multi-disciplinary team for planning and treatment needs of the patient
  • May run/facilitate support/psychoeducational groups
  • Explains policies, regulations, and programs
  • May be involved in integrated, multidisciplinary or community initiatives and may be housed at community based sites including serving as a liaison between Hennepin County and communities, community organizations, schools, health professionals and service providers and identifying needs and assist in resolving problems facing individuals and families regarding health and human service issues (including but not limited to housing, employment, financial assistance, and behavioral health)
  • Takes initial history, prepares psychosocial evaluation and care plans, including management of risk
  • Assists in the development of protocols for specific patient population
  • Is knowledgeable of and utilizes appropriate community resources
  • Assesses and diagnoses the psychosocial needs of the patients, caregivers, and families-how these needs are impeding the plan of care, as evidenced by documentation on the plan of care
  • Provides consultation to other professional health personnel regarding psychosocial issues and in-service education to health care team staff
  • Other duties as assigned but only after appropriate training


QUALIFICATIONS:

Minimum Qualifications:

  • Bachelor's degree in social work
Knowledge/ Skills/ Abilities:

  • Ability to communicate with patients and families under sometimes stressful circumstances
  • Experience with electronic health record or similar software program
  • Effective use of all Windows-based applications such as Outlook and Microsoft Office Suite products
  • Demonstrable cultural competence with a variety of socio-economic, racial and other elements of diversity
License/Certifications:

  • Unrestricted license by the Minnesota Board of Social Work as a Licensed Social Worker (L.S.W.)
